Emergencies are unplanned and may happen to anyone.  Rev. Alyssa, the Caring Team and Pastoral Care Partners encourage congregants to complete our Emergency Contact form. It includes vital information about you such as:

• Who should be contacted in case of an emergency?
• Who has a key to your home?
• Who is your Healthcare Power of Attorney?
• Do you have pets, or others living with you?

The information enables Rev. Alyssa to get in touch with your emergency contacts if need be. This information is kept in a secure location at the church and only Rev. Alyssa has access to your form. She may need to share your emergency contact information with emergency responders.
